Compassionate Heart has rapid hospital discharge processes to get you home safely when you are discharged from the hospital. On referral, a competent nurse will carry out a holistic risk assessment resulting in a support plan written on how well you want us to support you and the set-out agreed outcomes. The nurse will work with your doctor, pharmacy, therapist, and your family to ensure the continuity of your care on discharge. If your doctor has prescribed you medication, we will make sure that you have your medication and any other equipment in place that your therapist may have assessed as needed. You will be assigned a key worker who will review your support plan regularly or when your condition changes.
